Effective July 1, 2001 (or before, at the discretion of the bank), this notice is required to be distributed to all new and existing customers of Community State Bank at the time a relationship is established (initial), or to existing customers and on an ongoing annual basis thereafter.

Community State Bank collects nonpublic personal information about you from the following sources:

Information we receive from you on applications or other forms; Information about your transactions with us or others; and Information we receive from a consumer reporting agency (credit reporting agency).
We do not disclose any nonpublic personal information about you to anyone, except as permitted or required by law. If you decide to close your account(s) or become an inactive customer, we will adhere to the privacy policies and practices as described in this notice.

Community State Bank restricts access to your personal and account information to those employees who need to know that information to provide products and services to you. Community State Bank maintains physical, electronic and procedural safeguards that comply with federal and state standards to guard your nonpublic personal information.

If you have questions or need further clarification regarding our policy or practice regarding your nonpublic personal information, please contact us.

Please be advised that if you do desire us TO disclose nonpublic personal information about you to anyone, you must provide a specific written authorization to do so. It is strongly suggested that you contact an officer of Community State Bank before providing such written authorization to discuss the specific circumstances involved and to insure a clear understanding of what you expect the bank to do, and the bank's requirements to abide by your desires.

The Community State Bank is chartered under the laws of the State of Texas and by state law is subject to regulatory oversight by the Texas Department of Banking.
